This form is sent from the state office to the National FFA Center to specify who will be representing that state in the specific event at the national event.  If your state event occurs after July 7, please submit the certification form as soon as possible after the event is held.

This document lists the policies for career development events as determined by the Board of Directors.  Many of these policies are also listed on the introduction to the CDE handbook.

Information on how to request special needs accommodations.

The special needs request form must be submitted with the CDE certification form in order to provide adequate time to provide accommodations.  If accommodations are requested for a student who has been added to the team through the add/delete process, the request form must accompany the add/delete form.

All states that will be sending a participant in the speaking events must provide a judge from that state for the preliminary and semi-finals rounds. Any state may nominate an individual to act as a CDE assistant during any year.  This program in not required, but is an opportunity for states to learn about the events held at the national level.

This form allows states to request FREE plaques to present to their state winners.

Waiver, Release of Liability and Consent to Medical Attention Form.  This form is required for all participants in national career development events. States are advised to send waiver forms with certification forms whenever possible.  When that would cause a significant delay, states may send waiver forms after certification forms to allow teams to receive event information in a timely manner.

This form is sent from the state office to let CDE staff know how many teams to anticipate from each state.  States are invoiced $35 per team based upon the events they indicate chapters will be participating. Because supplies are ordered from this initial count, states may not be allowed to participate in an event which they declared whey would NOT be participating.

Use this application to nominate an agriculture education teacher for the Honorary American Degree.

Use this application to nominate an individual for the Honorary American Degree.  Must have state supervisor approval. 

Use this application to nominate an individual for the VIP award.

Use this application to nominate an organization, agency or business for the DSC.

State-level recognition will be given to those chapters qualifying for Superior Chapter recognition.  Each of these chapters receive certificates of recognition.

State-level recognition will be given to those chapters qualifying for state-level gold, silver, or bronze National Award spurs.

Use this form to verify those chapters eligible for National Star Chapter and Models of Innovation consideration.
